REVISED - Section C Form for Gradient Compression Garments

The PDAC has published a revised Section C form to gather necessary information for the proper coding of Gradient Compression Garments. Effective immediately all applications for code verification review (CVR) of Gradient Compression Garments should include this newly revised form and any related supportive documentation.
